03. Surcharge is 500 % of economy normal fare
04. Other importance informations to be inserted in reservation such as

1. Name and age of passenger
2. Nationality
3. Kind of sickness
4. Reason of travelling
5. Passenger need oxygen or not
6. Sponsor and or hospital at departure and arrival
7. Contact at departure and arrival
8. Ambulance arrangement at departure and arrival on passenger account
9. Passenger must be escorted by at least one person
10. Medical certificate release by Garuda Sentra Medika
